Keratin treatments have been the default fix for frizz since the mid-2000s, and the pitch has barely changed: hydrolysed keratin peptides get driven into the cortex under heat, then cross-linked with your hair's own protein so your curl pattern relaxes for months [1][3]. That part is real, and the chemistry behind it is well documented.

The safety story is where the category gets slippery. Since the NIH Sister Study linked frequent straightener use to uterine cancer in 2022 [7], and since a 2024 case series in the New England Journal of Medicine tied glyoxylic acid treatments to acute kidney injury [6], the old marketing language has stopped holding up. This guide walks through how each system works, which ones belong in your bathroom, and what to read on the label before you buy.

How a keratin treatment actually works

Two steps, in order. First the peptides have to get inside your hair. They are hydrolysed fragments, wool or feather keratin broken into short water-soluble chains, and size decides how far they travel. A 2020 penetration study tested several molecular weight fractions and found the low and mid-weight ones reached the cortex, while the heaviest fraction never made it past your cuticle [3].

Then comes the cross-link, and this is the step you are really buying. Classic systems use an aldehyde: formaldehyde or a releaser such as methylene glycol or glyoxylic acid reacts above roughly 400°F with the lysine, arginine, histidine and tyrosine side chains on the keratin backbone and forms methylene bridges [1]. Your curl pattern is not being relaxed. It is being chemically rebuilt, and that is why the result outlasts your next twenty washes.

Worth being precise about what this is not: a keratin treatment does not repair broken disulfide bonds. That is a bond builder's job [11]. Keratin lays a new protein layer over your existing structure, and washing, UV and heat strip it back over eight to twenty weeks. Keeping your hair permanently smooth means going back every three to five months.

What the penetration data supports

The 2020 study in the International Journal of Cosmetic Science is the reason anyone can claim keratin gets into the cortex at all: it compared hydrolysed keratins of different molecular weights and showed that only the smaller fractions penetrated, while the largest stayed on the surface and changed how the fibre felt rather than how it behaved [3]. Practical read: a product listing hydrolyzed keratin on your label says nothing on its own—the fraction matters, and almost no brand publishes it.

The four systems sold in the US, and what each one does

Start with the original, the one you have heard of. Classic Brazilian treatments cross-link with free formaldehyde, historically at 1 to 12 percent [2]. Oregon OSHA tested 57 samples in late 2010 and averaged over 8 percent, with a range of 6.4 to 10.8 percent [13]. Those numbers describe 2010 formulations, not what is on the shelf today, but they are why this category ended up on a regulator's desk in the first place.

Second: glyoxylic acid, marketed as the formaldehyde-free answer [5]. It carries its own aldehyde group and behaves like one under heat, releasing formaldehyde gas at the temperature your flat iron runs at. The bigger problem showed up in nephrology journals. Glyoxylic acid absorbs through your scalp, metabolises to oxalate, and oxalate crystallises in your kidneys [6].

Third: cysteamine and other aminothiols, which reduce the disulfide bonds and re-form them in a straighter shape, closer to a perm run backwards. No aldehyde, no formaldehyde release, but only four to eight weeks of hold and a sulfur smell you will absolutely notice. Fourth: hydroxide relaxers, a different product category entirely, with burn risk that keeps them out of most at-home smoothing kits [10].

The phrase formaldehyde-free is doing a lot of work

There is no legal definition behind it, which means nobody has to earn it before printing it on the box you are holding. A 2011 exposure study measured four professional smoothing products in use and found that some labeled formaldehyde-free pushed air concentrations past the OSHA short-term exposure limit of 2 ppm during the heat step [4]. In one Oregon lab test, a sample carrying a formaldehyde free label measured roughly 8.5 percent dissolved formaldehyde [13].

The 2025 systematic review holds the number that should change how you shop. Reviewing eight studies covering 34 patients and 36 episodes of acute kidney injury after glyoxylic acid smoothing, researchers found a median of two days between treatment and injury, oxalate crystals in the kidney tissue of 77 percent of cases, and—this is the part that matters at the shelf—20 of the 36 episodes involved a product labeled formaldehyde-free [9].

Everyone recovered. The review also found glyoxylic acid at 0.56 to 17.9 percent across the products it could analyse [9], which tells you the concentration going onto your scalp is essentially unknowable from your packaging. France's ANSES confirmed a national health alert on glyoxylic acid in straightening products in January 2025 [12].

The cancer signal sits alongside this. The NIH Sister Study followed 33,947 women and found that using straightening products more than four times a year was associated with roughly 2.5 times the incidence of uterine cancer—4.05 percent versus 1.64 percent by age 70 [7]. Small in absolute terms, and worth keeping in proportion. It is also the first epidemiological signal of its kind, and the reason the FDA started drafting a rule at all.

Read the INCI, not the front of the box

If the ingredient list contains glyoxylic acid, glyoxyloyl carbocysteine, methylene glycol, methanediol or a formaldehyde releaser such as DMDM hydantoin, the formaldehyde-free claim on the front describes the bottle at room temperature—not what leaves it at 450°F [8]. OSHA says the same thing more bluntly: products without formaldehyde or methylene glycol on the label can still expose you to formaldehyde [13].

What US regulators have actually done

Start with the thing almost nobody writes down: there is no federal ban. Formaldehyde, methylene glycol and glyoxylic acid do not appear anywhere in the FDA's list of prohibited cosmetic ingredients [18]. Kits containing formaldehyde are on sale to you right now—one Amazon listing carries the line "This product contains Formaldehyde" in the product title itself, at $89.99 [19].

Less than you would assume, and less than the headlines you have seen suggest. The FDA published its intent to propose banning formaldehyde and formaldehyde-releasing chemicals in hair smoothing products in October 2023, then moved the target repeatedly: April 2024, November 2024, March 2025, July 2025, and finally December 31, 2025, which also passed without action [16]. There is no statutory deadline forcing the agency's hand.

What the FDA does have is a consumer warning page describing formaldehyde release during the heat step and listing the releaser ingredients to watch for [8], plus exactly two warning letters issued in this category, to the maker of Brazilian Blowout and to Van Tibolli Beauty. That is the entire federal enforcement record.

State law is where something changed. California's Toxic-Free Cosmetics Act (AB 2762) banned 24 ingredients from cosmetics sold in the state as of January 1, 2025, intentionally added formaldehyde among them [14]. If you live in California, the shelf in front of you is already different from the one your friend in Texas is looking at. On the workplace side, OSHA's formaldehyde limits still apply in salons: 0.75 ppm over eight hours, 2 ppm short-term [13].

Salon or bathroom: what you actually trade

What you trade is not ingredient quality. It is heat control. Salons run professional irons with tight temperature regulation and multiple even passes per section, and the cross-link depends on that consistency [1]. At home, the kit supplies the solution and you supply the iron, which makes your iron's temperature accuracy the variable that decides your result.

The money is not close. Booking-platform data puts a US salon keratin treatment between $82 and $400 depending on the state, with New Hampshire highest and Utah lowest [15]; editorial estimates run wider, from $125 to $800 [17]. At-home kits on Amazon sit between $24 and $40: Kativa's Brazilian kit at $24.95 with 10,639 ratings, Kerazon's at $39.99 with 7,713.

Hold is the smaller gap: three to four months if you do it properly at home, against four to five in a salon. What you are really buying in a salon is ventilation, a trained hand, and someone else's lungs taking the exposure, which, given what the air measurements show [4], is not a small line item.

Matching the system to your hair

If your hair is tightly curled (type 4a-4c) or densely wavy (2c-3b), you need the highest cross-link density to read as smooth, which is why these hair types get the least out of gentle peptide-only systems. Coarse hair also holds heat differently, so your pass count matters more than your temperature dial.

If your hair is fine to medium and wavy (2a-2c), you have the opposite problem. Over-cross-link fine hair and you get the flat, plasticised look and the volume loss people mean when they say a treatment ruined their hair. Run bleached or fine hair at 390 to 410°F rather than the 450°F your instructions call for—the cuticle is already compromised [10].

If your hair is heavily bleached or has been colored more than once, run a bond-building routine before you go anywhere near a flat iron at smoothing temperature. Hydrolysed keratin measurably reduces strength loss in damaged fibre [11], but it is not a substitute for repairing the bonds that are already broken.

How the big US systems differ

Brazilian Blowout, the name you are most likely to recognise, runs two lines. The original works through methylene glycol, which releases formaldehyde when your iron hits it, and OSHA lists it by name among formaldehyde-releasing products [13]. The company states that its FREE line contains no formaldehyde, methylene glycol, formalin, glyoxylic acid or any formaldehyde derivative, and works inside the fibre rather than on the cuticle.

Cezanne is explicit that its Tri-Fusion technology is built on vegan glyoxylic acid together with hydrolysed keratin and quinoa protein. GK Hair's The Best lists glyoxylic acid second on the INCI, right after water, plus glyoxyloyl carbocysteine. Its Juvexin V2 is a plant protein from quinoa and pea, not keratin. Keratin Complex appears on the same OSHA list [13]; its safety data sheet reports air measurements up to 0.1 ppm.

None of this makes one brand safe and another dangerous, and you should be suspicious of anyone who tells you otherwise. It tells you which chemistry you are signing up for. And for the glyoxylic acid systems, the concentration is not something you can read off a box [9].

Four claims worth retiring

Keratin treatments repair your hair. They do not, and this is the claim that costs people the most money. They lay protein over the structure and smooth it; broken disulfide bonds need bond builders with bis-aminopropyl diglycol dimaleate or sh-oligopeptide-78 [11]. Smoothing and repairing are different jobs and the marketing has blurred them for a decade.

Formaldehyde-free means safe. It does not. The term is unregulated, it usually describes the concentrate rather than the gas produced at heat [8], and the substitute chemistry has its own documented injury profile [9].

Keratin makes hair thinner. It does not. A correctly done peptide treatment fills cuticle gaps and can slightly increase shaft diameter. What you are seeing is volume loss: your hair lies flatter against your head, and the total mass has not changed.

Anything with keratin in the name contains keratin. Not necessarily—GK Hair's Juvexin V2 is quinoa and pea protein. Plant proteins are not a scandal. But if you picked the product because you wanted keratin, you deserve to know what is in your bottle.

Where the evidence stands

On efficacy, the ground is solid. Low molecular weight keratin peptides demonstrably penetrate the cortex and improve mechanical properties [3], and a 2025 study on UV-exposed hair showed hydrolysed keratin significantly reduces the tensile strength loss that untreated fibre suffers [11]. The smoothing works, and it works for a reason you can look up.

On safety, the picture has gotten worse rather than better. The Sister Study [7] is the first epidemiological work with enough statistical power to flag the association. The NEJM case series [6], the ANSES alert [12] and the 2025 systematic review [9] together document a specific, mechanistically explained injury pathway for the ingredient that was supposed to be the safe replacement.

One honest gap, and it is the one that affects you most directly: nobody has published air measurements for at-home kits specifically. Every formaldehyde number in this article comes from salon settings or product analyses [2][4]. Your bathroom is smaller and worse ventilated than a salon, which suggests your exposure is not lower. That is inference, not measurement, and you should treat it as such.

The verdict

A keratin treatment is still the most effective way to keep frizz-prone hair smooth for months, and the science under the smoothing is sound. The safety case for the aldehyde systems is not. Those two things are both true, and the category has spent fifteen years hoping you would only hear the first one.

If you are doing this at home: take the peptide or cysteamine route, open every window you have, wear a respirator rather than feeling silly about it, and hold your iron to the temperature your hair can survive rather than the one printed on the instructions. And if you are pregnant, or you would be doing this more than a few times a year, the salon chair—or a different look—is the better call.

  1. Ventilate first, then set up

    Open every window, run an extractor fan if you have one, and put an N95 or better within reach. Gloves on, old clothes: the solution grips fabric and does not let go.

  2. Clarify twice

    Follow the manufacturer's processing time, usually 30 to 45 minutes, then blow dry all the way. The waxy feel afterwards is normal and not a sign you did something wrong.

  6. Flat iron with control

    Five to seven slow passes per section. The heat is what forms the bond—but drop to 390-410°F on fine or bleached hair rather than running the full 450°F [10].

  7. Wait, then switch your shampoo

    Depending on the product, 24 to 72 hours with no washing, no clips, no tucking behind your ears. After that, sulfate-free shampoo only, because sulfates strip the treatment out within a few washes.
